Dear Grade 6 Families, 

Forms from Camp Bournedale

Registration and health forms are due September 16, 2026 and should be submitted to the classroom teacher. Students have received  hard copies of these forms, however they can also be downloaded from the website if needed.

Payment

The cost of Camp Bournedale is $355.00.  Payment is due by September 16, 2026. All checks should be written to Wrentham Public Schools and submitted directly to your student’s homeroom teacher. Please use blue or black ink when writing checks. Please include your child’s name in the memo line of the check. If you need financial assistance, please contact Dr. Martes via email. 

Chaperones

Chaperone forms are attached. If you are interested in chaperoning, we ask that you complete the chaperone paperwork and submit it to your child’s classroom teacher by September 16,  2026.
